Preparing for your visit
In order for our doctors to understand your history and formulate the best treatment plan possible, we will request as much information as possible prior to your visit.
We ask you to fill out intake forms and offer 3 possible solutions.
- Option 1: Fill out the online forms with your computer or ipad/iphone using our Patient Portal. You will receive a link to the portal upon registering with our practice.
- Option 2: On the day of your first visit you can use our Kiosk to answer questionnaires and sign consent forms. Please arrive 30 minutes early to have enough time.
- Option 3: We will provide paper forms for you to fill out in the waiting room on the day of your appointment. Please arrive 30 minutes early to have enough time.
Please bring any pertinent records or test results. If you have had MRI scans please bring the CD disc and give to our front desk when you arrive so we can start to review.
COVID 19 SCREENING PROTOCOLS
Patient safety is our primary concern at Shore Spine & Spine. We are following strict CDC guidelines as it relates to the Covid-19 pandemic. Consequently, every patient and visitor to our office will be required to complete a COVID-19 screening questionnaire prior to entering. Mask wearing and appropriate social distancing are required as well.
Motor vehicle accidents
If you were involved in a Motor vehicle accident please provide your insurance claim number and declaration page from your policy.
Workers Compensation
If you were involved in a worker’s compensation injury, we will need your claim number and adjuster information.
